For some becoming healthy means losing weight. The numbers on the scale are not everything. If lowering your A1C to gain better control of your diabetes is achieved with healthy lifestyle changes, we consider that an achievement that should not go without celebration.

PRE-CONTEMPLATION
The first step that will help you change a habit. Whether you want to change a bad habit of unhealthy eating, lack of exercise, negative thinking or poor planning- this is the stage where you start to realize that you may want to change. Congratulations you are on your way!
2.
CONTEMPLATION
This stage begins the minute you realize you have a habit that affects you
and you know that it would be best to change it. This contemplation of change can last months, years or your whole life. If you have recognized a problem that you want to change, such as overeating, sedentary lifestyle choices, or lack of motivation, you can depend on Be Well to help you achieve your goal of change.
3.
PREPARATION
This is the stage when one really begins to change a habit. This stage is met with trials and failures, over and over. It really is hard to change a habit, but you know you need to change. You begin to look at what is holding you back. These challenges need to be stated and a plan created to help you move past them. This is part of our treatment process at Be Well. You are making progress!
4.
REALIZATION
There are noticeable achievements during this phase. Small changes are
better than no changes, and those small changes add up to accomplishing something great! During this phase, a strategy has been created and implemented, but it is important not to forget all the steps, otherwise failure may happen. The staff at Be Well will help keep you focused, reminded, and motivated of your goals.
5.
MAINTAINING THE NEW HABIT
To maintain change, you must integrate it into your everyday life with
confidence. This is accomplished by having new reinforcement habits. The staff at Be Well will help teach our patients new reinforcements and celebrate every achievement!
6.
RELAPSE
Relapse happens to even the most successful people. If this happens, you need to go back to step 1 and start again. This time, each step may come more easily because you have done this before. This is a time of reflection to figure out what led to the relapse and identify and/or eliminate risk factors. We are always here to help our patients get back on track and will reassure you to never give up. You will never be judged for failure, only embraced with encouragement for success.
